Industrial Accident: inadvertent motion of a steel extrusion tubing mill. The PLC control system had been retrofitted onto an existing mill to include display and control stations at several points. The issues involved the ability of stations not within line-of-sight to initiate operations of the mill during a routine maintenance procedure.
