Industrial Accident: Monitoring, control, and SCADA interface for an automatic tap changer installed on a high voltage transformer in a utility substation. The forensic investigation included overall design of the system as well as interface of the locally mounted monitoring unit to the utility's system-wide SCADA and control system.

Key issues included the performance of the monitoring system used to detect failure of the on-load vacuum bottle interrupters. Also important was the troubleshooting activity surrounding the explosion during which rapid tap changes occured when the ATC monitoring and SCADA systems were potentially not operating properly.
