Industrial accident: Motion of PLC controlled conveyor belt and band strapping machine during routine maintenance procedure. Operator was injured while loading metal strap material with conveyor in motion. Light sensor interfaces were wired to PLC logic for each conveyor segment. Forensic analysis involved multiple segment conveyor coordination during the service procedure that relied on local control station rather than lockout-tagout (LOTO).
