Industrial accident: an unexpected cycle of a mechanical power press with a two-hand control reliability interlock and electro-mechanical control system. The issues involved modifications and maintenance performed on the system after the initial installation.

The control system design was analyzed in regard to OSHA 29 CFR 1910.217 and related industry standards such as ANSI B11. Technical aspects of the control system design were tested for anticipated failure modes as well as unexpected single and multi-point failures. Testing was conducted both on-site and in a controlled environment.
