Industrial Accident: Pipeline Leak Detection Failure

Industrial accident: undetected leak on a liquid pipeline fitted with a PLC and computer-based leak detection system. The system used radio communications to monitor pressure, temperature, and flow at key points along the pipeline. This data was transmitted via SCADA and accumulated within a PC-based leak detection subsystem. The forensic analysis involved isues of systems integration, reliability of field devices, radio links, and data integrity in a realtime leak detection system.
