Industrial Accident: fatality within a lead acid battery recycling machine. The process used automated grinding and separating equipment controlled by a single panel that contained a design flaw in a timing circuit. Inadequate lockout tagout procedures permitted cleaning of the machine without isolation of hazardous energy as required by OSHA. An unknown control reversal problem had occurred when the panel was relocated.
Although the Emergency Stop circuit had been operated, unexpected motion occurred during maintenance because of the timing issue. The Estop status, combined with the control reversal problem hampered attempts to free the trapped worker from the moving hydraulic ram.
