Industrial Accident: worker injured while performing non-routine service on a foundry conveyor. Employee used the emergency stop button (E-Stop) rather than the prescribed lockout-tagout (LOTO) procedure to halt the conveyor. The E-Stop disabled normal PLC operation, but did not isolate all electric and hydraulic energy. The isolation of hazardous energy could not be verified without functional PLC controls, resulting in a release of high pressure hydraulic fluid.
